With the signing of this Release and Consent, I accept and agree to the following conditions: 1. Release from Liability: I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me during participation in Dancecrush classes, rehearsals, performances or activities. I also exempt, release, and indemnify Dancecrush, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, or property which may arise out of or in connection with participation in any classes or activities conducted by Dancecrush. I further hereby voluntarily agree to waive our rights and that of any heirs and assigns to hold Dancecrush, its owners, agents, volunteers, assistants, employees, independent contractors, guest artists, faculty members, and/or student liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. 2. Use of Likeness: Permission is granted to Dancecrush to use photographs and/or video of any performances, lessons, or other for promotional purposes via Print/Video/Social Media or any other method of advertisement for the event, and no compensation will be due to Dancecrush for usage of this material.
